You understand diverse health conditions and how to navigate the healthcare system. You’re skilled at helping people make sense of healthcare information and care plans, and you’re passionate about advocacy and client‑centred care.

If this sounds like you, consider Wabano as your next career opportunity.

At Wabano, you will provide trauma‑informed, intensive case management to Indigenous people with complex health and mental health needs. You’ll work in a supportive, values‑driven environment where you are valued, challenged, and supported.

About Wabano

The Wabano Centre for Aboriginal Health is an award‑winning Indigenous Health Access Centre providing holistic, culturally grounded health services to First Nations, Inuit, and Métis people living in Ottawa—one of Canada’s fastest‑growing and most diverse urban Indigenous communities.

Wabano is a fast‑paced, client‑focused organization that models Indigenous values of balance, respect, and community interdependence.

Position Summary

The Care Coordinator provides high‑quality, client‑centred case management to individuals living with complex medical and mental health needs. Using an intensive case management approach, the Care Coordinator works collaboratively with clients, their personal supports, and internal and external care providers to create and implement coordinated care plans rooted in the client’s priorities.

The Care Coordinator plays a key role in improving clients’ experiences within the health and social service systems through advocacy, creative problem‑solving, and accountability.

Key Responsibilities

  • Manage a caseload of clients with complex needs in collaboration with internal and external health and social service providers
  • Develop and implement integrated care plans that coordinate services within Wabano and across partner organizations
  • Facilitate case conferences with clients and their support teams to enhance communication and continuity of care
  • Advocate for clients to improve their experiences navigating health and social service systems
  • Provide education and coaching to clients and their supports, including chronic disease self‑management, coping strategies, communication skills, and other life skills
  • Accompany clients to appointments, when needed, to ensure their voices are heard and they receive appropriate, high‑quality care
  • Monitor progress and adjust care plans based on clients’ goals, needs, and strengths

Qualifications

Don’t disqualify yourself! You may not meet every qualification, and Wabano values and recognizes the lived experience of Indigenous people.

  • Post‑secondary education in Social Work, Nursing, Health Sciences, Public Health, or a related field
  • Minimum of 3 years’ experience in a related role
