Your Opportunity As the Care Finders Coordinator, you will work with a small team of Care Finder Support Workers and provide specialist, intensive support to older Australians who face barriers accessing aged care or community services. Working across multiple locations, you’ll support the delivery of the Care Finder Program, ensuring engagement with clients, collaboration with Primary Health Networks and community organisations, and effective service navigation for vulnerable people. This is a hybrid role with a mix of work from home and office setup. With empathy, initiative and professionalism, you will: Respond promptly to general enquiries and intake applications Engage with clients, carers and intermediaries to build rapport and identify needs Support clients through My Aged Care, assessment pathways and service navigation Maintain accurate and timely documentation within the CRM Hold a partial client caseload while providing guidance to Care Finder Support Workers Collaborate closely with Primary Health Networks and participate in Community of Practice meetings Build and maintain strong partnerships with local organisations and referring agencies Promote the service through local engagement and community activities Provide leave coverage, assist with reporting and oversee risk and safety considerations Champion Anglicare’s Christian vision, mission and values through leadership and conduct Contribute to continuous improvement, policy compliance and program enhancement What You’ll Bring You are passionate about making a difference for older Australians, with the leadership skills and sector knowledge needed to guide staff and support people who require intensive service navigation. You will also need: Diploma of Community Services or equivalent qualification Experience supporting and/or leading a team Previous experience in Home Care, Aged Care or Community Services Strong understanding of aged care systems and resilience Relevant tertiary qualifications for the professions of Psychologist, Social Worker, Counsellor, Nurse or equivalent and current membership to professional body (AHPRA, AASW or Australian Counselling Association or equivalent) Experience in working with people with complex mental health challenges/aged care Proficiency using digital systems and client management applications Driver license and self-owned car You will thrive in this role if you bring: Warm, clear and confident communication skills Strong relationship-building abilities Commitment to Anglicare’s Vision, Mission and Values A proactive, compassionate and client‑centred approach Why Anglicare Sydney?
