Description Care Guide $29.96 -$40.45 This role leads and coaches the Certified Nursing Assistant (CNA) and Clinical Support Nurse teams at Woodland Park Complete Living Care homes. Key responsibilities include interviewing and retaining staff, and providing support for the onboarding process and ongoing development of CNAs and Nurses. Supervisory Responsibilities As the self-managed work team's direct supervisor (Guide), this position entails coaching, directing, and monitoring team members, either directly or through team coordinators. The supervisor will also evaluate the performance of CNAs and Nurses, ensuring high-quality services and enhancing the quality of life for residents. Audits and Admission Management The role involves conducting various audits on blood sugar, vaccinations, TB, anticoagulant monitoring, lab kardex, bowel and bladder management, and catheter care. Additionally, the supervisor will manage admissions, including order entry and admission assessments. Pre-Admission Process Leading the pre-admission assessment process, which includes briefing, coaching, and preparing CNAs and nurses for incoming residents, is crucial. Education and Incident Management An essential aspect of the role is providing education and leadership during transitions, including grief support. The supervisor will also lead investigations of accidents and incidents, handling initial reports and gathering witness statements. Quality Assurance and Collaboration Follow-up initiatives related to Quality Assurance and Performance Improvement (QA/PI) will be part of the responsibilities, including charting and coaching on Performance Improvement Plans (PIPs). This role requires collaboration with Supportive Living and marketing service lines to establish a service delivery system that aligns with the mission, vision, and values of VMRC. Requirements Graduate of an Accredited School of Nursing with a current license to practice nursing as an RN or LPN in the Commonwealth of Virginia (BSN preferred). Past demonstrated supervisory, coaching, and support skills with personal care staff. Previous experience in Long Term Care/SNF required. MDS, RAI, and care plan experience is preferred but not required. Possess the ability to communicate effectively while maintaining a professional working relationship with the Director of Nursing, physicians, employees, families, and the public. Knowledgeable and passionate about geriatrics and long-term care. Ability to supervise, coach, coordinate, and effectively lead the activities of others. Must possess a command of the English language, both written and spoken; able to communicate orally, in writing, and through presentation effectively. Work collaboratively throughout the entire organization to effectively engage staff, residents, family members, and other stakeholders. Possess computer experience with proficiency in Microsoft Office suite (Word, Excel, PowerPoint, and Outlook) for operations, report generation, and presentations. Self-motivated, organized, and willing to be flexible to accommodate changing priorities. Proven ability to work both independently and in a team-based environment with minimal supervision.
