Responsibilities

ABOUT THE ROLE

We are looking for a dedicated and compassionate Care Home Assistant Practitioner (CHAP) to join our team on night OR day shifts. This role is ideal for a skilled and experienced healthcare professional who is CHAP trained and passionate about delivering high-quality, person-centred care.
As a CHAP, you will support Registered Nurses by carrying out a range of clinical interventions under supervision. Your role is key in ensuring our residents receive safe, effective, and compassionate care.

KEY RESPONSIBILITIES

  • Support Registered Nurses in delivering high standards of care
  • Administer medication, wound care, catheter care, and other clinical tasks (once trained and assessed as competent)
  • Assist with clinical procedures such as blood sugar monitoring, PEG/PEJ care, and nursing observations
  • Record vital signs, food/fluid intake, and maintain patient records accurately
  • Provide personal care and assist residents with mobility and activities of daily living
  • Promote dignity, independence, and resident choice in all aspects of care
  • Support care staff through training, mentoring, and supervision
  • Contribute to care planning, risk assessments, and health & safety practices
  • Maintain confidentiality and adhere to company policies at all times
