Franciscan Health Crown Point 12750 S Francis Dr Crown Point, Indiana 46307 The Care Management Assistant performs secretarial or clerical functions to support the administrative operations of an assigned department. The Care Management Assistant supports the Care Management Department by providing clerical and administrative relief for the department's Care Managers and Social Workers. This position assists with the operations of care delivery by arranging post-discharge services, managing Medicare correspondence, providing clinical updates to commercial payers, as well as communicating necessary discharge planning updates to patients, families, and hospital care team staff. WHAT YOU CAN EXPECT Schedule: Monday - Friday, 7:30am - 4pm Obtain status details regarding care authorizations and supervisory visit requirements from insurance payers Communicate Medicare rights to patients and their family members if applicable; issues written Medicare-related communications to patients, including status changes, and follow-ups Prepare and provide patient care teams with detailed written reports describing the nature of the care required, with supportive documentation from patient records, community resources, services, and equipment needed to assist in meeting patient and family post-discharge needs. Coordinate transportation, post discharge and prepare discharge packets for post-acute providers.
