Job Description Participates in community outreach and enrollment activities. Provides assistance/ guidance to members, family members and caregivers. Serves on an inter-disciplinary team that provides care coordination, managing the medical, social, and behavioral health needs of enrolled members. Job Responsibility Performs outreach to program candidates among medically underserved populations within the community. Completes related assessment; performs administrative assessment of candidates’ eligibility for social services and care management programs. Obtains required signatures and maintains required documentation in accordance with program policies and procedures and state/federal regulations. Assists members in scheduling appointments for consultations, diagnostic testing, program meetings, etc. Collaborates with clinical and non-clinical team members to maintain proper workflows, procedures and documentation. Develops and generates reports summarizing program activities, as required by management. Participates in team meetings to provide non-clinical input regarding member status and activities. Participates in developing relationships with referral sources to facilitate enrollment. Serves as a point-of-contact to program members, family members and caregivers; encourages use of program resources to maximize wellness and independence. Performs related duties as required. Job Qualification High School Diploma or equivalent required. 1-3 years of relevant experience, required. Bachelor's or Master's degree in relevant field strongly preferred. Health home or care management experience strongly preferred.
