Key Responsibilities

  • Build genuine, trusting relationships with clients, families, and representatives, becoming their go-to support
  • Conduct regular care reviews (in person or via phone) to ensure services continue to meet client needs and expectations
  • Collaborate with caregivers, families, and health professionals to ensure care plans are personalised and effective
  • Maintain strong community connections by engaging with local services, health providers, and referral networks

About you

  • A tertiary qualification in a health-related field (e.g. social work, allied health) with at least 1 year of experience working with seniors and Home Care Packages
  • Excellent organisational skills with the ability to juggle priorities and adapt to changing needs
  • You thrive in a team environment and are willing to offer assistance when needed.
  • Strong communication skills, both written and verbal, with proficiency in Microsoft Office
  • A valid Australian Driver’s Licence and the flexibility to travel when required
