Right at Home Gosford are seeking a qualified and experienced clinical Care Manager - EN/RN to help our clients live empowered and independent lives at home. Our mission is to improve the quality of life for those we serve.

PREFERRED SKILLS

  • Sound understanding of the Aged Care and Disability government funding systems
  • Demonstrated experience supporting staff
Responsibilities

YOUR NEW ROLE WILL BE RESPONSIBLE FOR:

  • Meeting with clients and their families and coordinating their needs within their funding model
  • Conducting client care assessments and creating care plans to best meet client requirements
  • Handling information requests, feedback, and general enquiries
  • Scheduling services for our clients by rostering care workers dependent on their qualifications and availability
  • Ensuring all documentation is accurately and comprehensively completed in Right at Home databases and software programs
  • Performing some community nursing care as needed

TO BE SUCCESSFUL IN THIS ROLE, YOU WILL NEED:

  • A current AHPRA Registration
  • Qualifications and experience in Nursing
  • Highly developed verbal and written communication skills
  • Strong organisational, problem solving and time-management skills
  • Proficient computing skills
  • Current Australian National Police Check & Working with Children Check
  • Current CPR certificate
  • Full Driver’s license and your own reliable, insured vehicle
  • The right to work in Australia
