Job Description: Broadstep Behavioral Health is hiring a Care Manager Extender for our Tailored Care Management (TCM) team in Jacksonville, NC. The Behavioral Health and Intellectual/Developmental Disability (I/DD) Tailored Care Management (TCM) model reflects the goal of whole-person care management in NC Medicaid Managed Care. Provider-based care management promotes integrated care and offers beneficiaries a choice in how they receive care management. Schedule: Day Shift, M-F, 8am-5pm (with some flexibility) Job Location: This role is based out of our Jacksonville, NC office with estimated travel one day per week to either Onslow, Craven, Carteret, or Pender county with mileage reimbursement. Job Duties: Performing general outreach, engagement, and follow-up with members Coordinating services and appointments (e.g., appointment/wellness reminders, arranging transportation) Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing Collaborating with the primary Care Manager and other team members on current needs and status of the member Placing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services Participating in care reviews Supporting the Care Manager in assessing for and addressing unmet health-related resource needs Consistent communication and coordination efforts with Care Manager and Care Manager Supervisor to decrease duplicative and inappropriate outreach to members Other duties as assigned by Care Manager Supervisor based on needs of the team Required Education and Experience: At least 21 years of age A high school diploma or equivalent (e.g., GED, certificate of completion); A valid driver's license, auto insurance, and reliable transportation And meet one of the following requirements: Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system. or Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist. or A parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (note that a parent/guardian cannot serve as an extender for their family member). or Has two years of paid experience performing the types of functions described in the “Extender Functions” section below, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
