We are currently hiring a Care Manager. As a Care Manager, you will assess and monitor the needs of the consumer and implement a service plan which is following the Executive Office of Elder Affairs and Elder Services of Cape Cod and the Islands Home Care Program regulations and contract obligations. This is a full-time position, Monday- Friday, 34 hours/ week plus unpaid one hour lunch each day. The pay rate for this position is $29.29 per hour. What do you need to be considered for this position? * Ability to collaborate, take initiative, provide/ receive feedback. * Ability to communicate and work with the elderly and community resources serving the elderly population. * Excellent computer skills required. Ability to work independently and professionally. * Valid MA driver’s license and car required. * Prolonged periods sitting at a desk and working on a computer. * Must be able to lift up to 15 pounds. * Travel Required daily throughout the service area. Travel to training and meetings as needed. Responsibilities

The Care Manager will assess and monitor consumer needs and implement a service plan in compliance with Executive Office of Elder Affairs and program regulations. This involves coordinating services and care for elders in Barnstable, Dukes, and Nantucket Counties.
