Description We are excited to announce an opening for a Care Manager at our Kissimmee BVL office! In this role, you will perform both clinical and administrative duties under the direct supervision of our Physicians. Your warm and welcoming demeanor will help put patients at ease throughout their visit, as you provide support during diagnostic examinations and treatments, administer medications, draw blood, and facilitate communication between patients and physicians. Additionally, you will take pride in the vital role you play in helping our doctors and staff deliver excellent care. Key Responsibilities: Manage cases via phone, including assessments, education, and symptom monitoring. Analyze data for Disease Management enrollment and apply critical thinking in care planning. Conduct holistic patient assessments and coordinate interdisciplinary care. Collaborate with physicians for resource utilization and clinical management. Maintain current knowledge of disease processes and treatment options. Ensure accurate documentation and compliance with guidelines. Communicate effectively, support team goals, and drive performance improvement. Educate members and staff on the DM Program; present updates to management. Contribute to NCQA and URAC accreditation efforts. Requirements High school diploma or General Education Degree (GED). 1 year of experience as a Medical Assistant. 1 year of related experience in outpatient clinic, hospital setting, insurance, medical records, billing, or related area preferred. Six to twelve months of data entry experience required. Intermediate-level MS Excel skills are required. Working knowledge of medical terminology preferred. Experience with hospital portal systems preferred. Effective interpersonal skills are required. Good organizational and time management skills. Effective Communication Skills (Written and Oral) Should possess good Phone Skills. Must be Detailed Oriented.
