KEY REQUIREMENTS (IN ADDITION TO THE POSITION DESCRIPTION ATTACHED):

  • Minimum 3 years’ experience as a Care Manager in the aged care sector
  • Proven clinical and management expertise in a Memory Support Unit setting
  • Strong leadership, communication and team collaboration skills
  • Flexibility to transition to a full-time position

QUALIFICATIONS

  • Tertiary qualification in Nursing is essential. Qualifications in Social Work or Allied Health at a Bachelor level or higher are also considered.
  • Current AHPRA registration highly regarded

SKILLS & EXPERIENCE

  • Extensive experience in aged care management, with a broad clinical background and expertise in residential care
  • Demonstrated leadership and mentoring skills to develop, support and motivate staff
  • Comprehensive understanding of the Aged Care Quality Standards and contemporary clinical care practices
  • Strong knowledge of aged care legislation, clinical governance, and quality and risk management frameworks
  • Proven ability to oversee and implement clinical care systems, including continuous improvement and accreditation processes
  • Excellent communication skills, including the ability to liaise effectively with residents, families, staff, and external stakeholders
  • Sound judgement and ability to manage incidents, complaints, and issues proactively
  • Intermediate to advanced computer skills across a broad range of software applications
  • Demonstrated ability to drive change and innovation in aged care settings
  • Ability to meet compliance requirements of the role, including:
  • Current First Aid and CPR certificate
  • Valid driver’s licence
  • Evidence of current flu and COVID-19 vaccinations

Responsibilities

ABOUT THE ROLE

The Care Manager at MiCare is an experienced and motivated leader responsible for ensuring holistic, person-centred care and support is provided to elders in MiCare’s residential services. This role plays a crucial part in supporting the operational and strategic management of the residential service, including all clinical, customer and quality outcomes related to care delivery.
The Care Manager ensures care reflects individual preferences and culturally appropriate practices while assisting clients to understand and access the level of support they require. You will work alongside the Residential Manager, fostering a collaborative approach to quality service delivery that meets the needs of all stakeholders, particularly our elders.

CARE MANAGEMENT RESPONSIBILITIES INCLUDE:

  • Ensuring resident care and services are consistent with the Aged Care Quality Standards, using a multidisciplinary approach to address gaps
  • Familiar with the new Aged Care Act and strengthened Aged Care Quality Standards, with an understanding of key reforms in consumer rights, provider responsibilities and quality care delivery
  • Leading, mentoring and educating staff to ensure quality services are delivered to residents
  • Overseeing clinical governance and implementing clinical care needs and systems
  • Leading and contributing to quality improvement, accreditation and continuous improvement initiatives
  • Monitoring the effectiveness of equipment, supplies and practices to ensure compliance with MiCare policies and procedures
  • Managing incidents, complaints, risks, and issues effectively and transparently
  • Engaging in innovation, research and partnership opportunities to ensure MiCare’s services remain contemporary and leading-edge
  • Supporting the development and review of organisational policies and procedures
  • Providing regular reporting on key performance indicators (KPIs) and contributing to financial sustainability through a strong understanding of the Australian National Aged Care Classification (AN-ACC) funding model, including accurate resident classification, care minute targets, and alignment of staffing resources, as well as effective consumable cost control.
  • Supporting residents’ social inclusion, spiritual and cultural needs while fostering relationships with families, staff, volunteers, and external service providers
