Paragon Home Healthcare are looking to recruit a dedicated and driven Care Manager. As a Client Care Manager, you will be an integral part of our team, delivering high-quality complex and specialist care and supervising carers to a caseload of clients. You will be responsible for the clinical aspects of care and training. Working closely with the team, you will support the delivery of high-quality homecare. You will report to the Field Supervising Manager and will be supported by the Clinical Director in the delivery of our nurse-led services. You will be based 2 days in the office while the rest will be working in the field and in Client’s home
Location: Richmond Head Office (minimum 2 days per week) & Client Homes (travel to various locations usually in and around the South East)
Starting Salary: £28,000 - £34,000 per annum depending on experience
EXPERIENCE AND ATTRIBUTES

· Proficient in spoken and written English. Health & Social Care Level 4 and above or equivalent.
· Confident to drive in UK with valid Driving license, preferably with access to own car

· Have a can-do and flexible working style and able to work well under pressure.
· Proficient in using a range of computer and app systems.
· Adept at using new apps and quick to learn how they work together.
· Excellent organisational and communication skills.
· Comfortable in people’s homes and able to adapt to a non-clinical environment.

JOB PURPOSE
· Case Manage a group of Care Clients
· Deliver training to staff in practice, assessing competency using a structured approach.
· Supervise the delivery of high-quality care for your caseload of clients in their homes.
· Provide mentorship, supervision, educational, and practice support to your care staff teams in their places of work.
· Actively participate in the direct care of your clients in their homes, providing safe, effective, high-quality, and compassionate care.
· Be part of the on-call rota as required.·
