How you make a difference:
The Care Manager is responsible for the overall tracking, management, and timely care provisions for patients with chronic medical conditions. This position works closely with on-site physician and mid-level providers to ensure chronically ill patients are seen on a routine and scheduled basis dictated by their disease process.

Key Responsibilities:

  • Maintain a comprehensive log for all chronically ill patients, including appointment schedules, diagnostic testing, and disease details. Prepare patients for chronic care visits, ensuring all required testing is completed and results are available for the provider.
  • Assist patients during visits as directed by physicians or mid-level providers, provide relevant education, and ensure special needs are addressed on a patient-specific basis.
  • Schedule off-site specialty consultations, present recommendations to on-site providers, and ensure corresponding orders are fulfilled.
  • Maintain chronic care statistical data, review aftercare recommendations post-ER or hospitalization, and conduct medication compliance studies, ensuring all reports are submitted as requested by the Medical Director.
  • Collaborate with the provider team and Director of Nursing to optimize facility resources for chronic care patients and ensure compliance with National Commission on Correctional Health Care standards.

Qualifications & Requirements:

Education

  • Must be a graduate of an accredited school of nursing

Experience

  • Three (3) years of relevant clinical experience is required. Prior medical/surgical or emergency/trauma experience is preferred. Previous corrections experience preferred.

Licenses/Certifications

  • Currently licensed as a Registered Nurse in the state within which services are provided and possess and maintain current CPR certification.
