Description Brattleboro Memorial Hospital / Southern Vermont Health Services Corporation Job Description Job Title: Care Manager RN Job Code: 260 Hours: 0800-1700 FTE: 1.0 (40 hours/week) Weekend Schedule: Holiday Schedule: No Call Schedule: No FLSA Status: Non-exempt Department Name: Care Management Department Code: 7700 Location: Brattleboro, Vermont JOB SUMMARY: In accordance with Care Management, Discharge Planning, and Social Service policies, the employee monitors and coordinates the plan of care; identifies and evaluates patients’ individual needs; and assists patients, families and health care professionals in addressing these needs and in developing a plan for post-hospital care. The employee also serves as an advocate for patients and families and as a resource and educator for medical staff and hospital personnel. DEPT. SCOPE OF SERVICE: The Quality Management Department has a hospital-wide scope of service in quality improvement, including patient satisfaction and complaint resolution, service excellence, risk management, infection control, and is the contact for participation in state and regional/national quality projects. The scope of service for peer review, credentialing and re-credentialing encompasses BMH Medical Staff practicing within BMH. Care Management/Utilization Management are hospital-wide in scope. The Care Managers/Social worker coordinate with external agencies to meet patient care needs as the patient transitions to an after-hospital care environment. The Quality Management Department also coordinates the Joint Commission compliance of the hospital. Requirements QUALIFICATIONS: Education: Graduate of NLN accredited school of nursing. Experience: Requires recent experience in Care Management, and in similar, or in other progressive jobs. One year experience in Med-Surg nursing, public health nursing, or discharge planning experience required. Psychiatric nursing is of additional value. Licensure/Certification: Vermont state RN license required. Remains current with continuing education/CEU requirements Mental Efforts: Requires ability to read and write. Requires hand-eye coordination and manual dexterity. Requires use of office equipment such as typewriter, computer, etc. Requires corrected vision/hearing to normal range. May require working irregular hours. Must have good organizational skills, ability to work independently, and ability to deal effectively and compassionately with patients, families and co-workers. The listed range represents the pay scale reasonably expected for a successful candidate based on factors such as relevant experience, education, internal equity, and market competitiveness.
