JOB SUMMARY

As a Care Manager RN, assess and organize patient care through treatment stages, following guidelines. Manage benefits, oversee services, act as a case manager and trainer to achieve optimal results. Effective care management helps minimize unnecessary resource use.

QUALIFICATIONS

  • A minimum of an Associate’s degree is required.
  • The applicant should have at least two years of professional experience.
  • The applicant should be a licensed Registered Nurse.
Responsibilities
  • Conduct thorough assessment, implement care plan, collaborate with team for evidence-based high standard of care.
  • Creating patient care plans that focus on interventions, treatment goals, and self-management for the patient’s benefit.
  • Examine the health needs of patients and families, then provide necessary care to meet those needs.
  • Assisting patients with self-management using education, personal visits, and telephone interactions, inspiring patients to stick to their care plans.
  • Researching resources for patient needs; coordinating programs for correct service delivery. Providing referrals to help patients.
  • Being proactive in identifying problems and solutions in coordinating care to provide the best possible options.
  • Acting as a bridge between healthcare providers to ensure smooth communication and coordination among different facilities.
