Job Title: Care Manager (Pharmacy Tech Training Provided) - Entry level Location: Southfield, MI (Hybrid/Remote after training and meeting performance metrics – ~90 days)  Job Type: Full-Time Schedule: * During training: 9:30 AM – 5:00 PM * After training: Flexible options: * 8:00 AM – 4:00 PM  * 9:00 AM – 5:00 PM  * 10:00 AM – 6:00 PM  * Or 8:00 AM – 6:00 PM (4 days/week)  * Monday–Friday (no weekends unless opting for 4-day schedule)  Transform Patient Care with EPIC Health At EPIC Health, we’re not just improving healthcare—we’re redefining it. As a Care Manager, you’ll play a vital role in supporting patients with medication management, care coordination, and overall health outcomes. No pharmacy technician license? No problem. What You’ll Do Medication Support & Coordination * Review and document patients’ medication histories for accuracy and completeness.  * Assist in coordinating medication refills, prior authorizations, and delivery services. * Support pharmacists and providers in identifying discrepancies or concerns.  Patient Education & Engagement * Educate patients on medication use, adherence strategies, and general care plans.  * Serve as a primary point of contact for patient questions and support.  * Proactively follow up with patients to ensure adherence and continuity of care.  Care Management & Transitions * Support patients through transitions of care (discharges, new diagnoses, etc.). * Coordinate across providers, pharmacies, and care teams to ensure seamless care.  * Identify and address barriers to care, improving access and outcomes.  Documentation & Systems * Maintain accurate, up-to-date patient records in the EHR system. * Document all patient interactions, interventions, and care coordination efforts. Compliance & Quality * Ensure adherence to HIPAA and healthcare regulations. * Participate in quality improvement initiatives focused on patient outcomes and safety.  What You Bring Required Qualifications * Experience in healthcare, patient care, patient coordination, or similar healthcare roles. * Strong communication and interpersonal skills. * Ability to manage multiple patients and tasks in a fast-paced environment.  * Comfortable using technology, including EHR systems (training provided).  Preferred Qualifications * Experience in long term care facilities, hospital settings, medication management, or chronic disease management. * Previous experience working with high-risk or complex patient populations. * Interest in obtaining a Pharmacy Technician license (training and support provided).
