SUMMARY

We are seeking a Care Navigator to join our team at Forbes Pharmacy and Blue Sky Professional Services. As a Care Navigator, you will serve as both a care provider and a team lead. When appropriate, you’ll assess client needs, review care documentation, conduct quality control visits, and step in to cover care shifts when needed. This role requires flexibility, a warm and encouraging presence, and strong leadership skills.

WHAT WE’RE LOOKING FOR:

  • Experience as a Care Aide or Internationally Trained Nurse
  • Demonstrated reliability and professionalism
  • Strong interpersonal and problem-solving skills
  • Leadership qualities and sound judgment
  • A valid driver’s license is required for the position (reliable vehicle preferred)

Responsibilities
  • Provide in-home care to clients when needed
  • Prepare written communications and administration tasks
  • Conduct regular quality control visits and assessments
  • Review communication binders and provide feedback
  • Mentor and support other caregivers
  • Step in to cover urgent or last-minute care shifts
  • Coordinate care in collaboration with our leadership team and may include clients, families, and team members
