We are currently seeking a Resident Support Specialist to join our Care Navigator team. What You’ll Do: Support residents’ independence and mobility while addressing emotional and social needs. Create personalized engagement opportunities based on resident interests. Provide transportation and accompany residents to meals or medical appointments. Assist with moving residents to various levels of care, as needed. Assist with long-term care policy claims and communicate with families and care teams. Observe and report changes in resident condition and document care accurately.
