Your Role as a Care Partner You’ll deliver this role by: Developing and reviewing personalised care plans that support customer goals, wellbeing, and independence Coordinating services and managing customer budgets within approved funding parameters Building trusted relationships with customers, carers, families, and health professionals Monitoring customer wellbeing, changing needs, and potential risks to ensure continuity of care Applying wellness and reablement approaches that encourage confidence and independent living Supporting customers to make informed decisions and navigate aged care services and supports Maintaining accurate documentation, care notes, and compliance with aged care standards and legislation Promoting safe, high-quality, and culturally inclusive care that respects each customer’s individual needs and preferences What We Offer Salary Packaging: As a not‑for‑profit employer, we offer generous salary packaging benefits that help maximise your take‑home pay.
