This role ensures that care and support services are planned, coordinated, and delivered effectively and within budget. The Care Partner works collaboratively with consumers, families, and service providers to develop and implement individualized care plans that promote independence, wellbeing, and quality of life. Through comprehensive assessment, planning, and ongoing monitoring, the Care Coordinator ensures that each consumer receives appropriate, timely, and cost-effective support aligned with their goals and preferences.
