The day-to-day responsibilities

• Establishes rapport with care recipients and their family/representatives. 
• Manages a dedicated portfolio of clients, meeting all monthly, quarterly, and annual review requirements. 
• Responds to deteriorating health by reviewing care needs and services. 
• Identifies vulnerable clients and ensures their care plan responds to needs associated with vulnerability.
