About the role 

You’ll manage a portfolio of clients across South East Melbourne, including NDIS participants and older Australians receiving aged care services, including Support at Home. 

You’ll spend time in client's homes and local communities, getting to know their needs and coordinating the right support. You’ll be their point of contact, helping ensure their care is safe, personalised and aligned with their goals and funding requirements.
