EXPERIENCE:

  • Knowledge of the aged care industry/issues or Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles and the Aged Care Quality Standards – Essential
  • Previous experience in Aged Care or related discipline – Desirable
  • Easily adaptable to new software platforms

As a Care Partner you will be responsible for:

  • Building strong and meaningful connections with clients and their family
  • Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home
  • Interacting and providing professional health and clinical care advice and direction to a team of home care workers
  • Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction
  • Ensuring all practices are consistent within policy and legislative frameworks
  • Maintaining up to date client records and information
