Company Description Care Partner Greater Sydney Team Central Coast/ Northern Sydney Job Description About the role: In this role, you will manage a caseload of community-based elderly individuals with Home Care Packages in Blacktown & surrounding areas. You will assess their needs, develop personalised care plans and manage budgets in accordance with program guidelines, to ensure each customer's unique requirements are met. You'll also onboard new customers and proactively manage existing ones with a person-centred approach. Collaboration with our Clinical, Scheduling and Service Delivery teams will be crucial to delivering comprehensive care. This is a 12-month fixed term contract, and you will have the flexibility to manage your own schedule each week, while working autonomously from home, our Bella Vista office and within the community. As you will regularly visit customers in their homes, you will need to be based close to, and willing to travel, within this region. About you: To be successful in this role, you will have: Ideally proven experience in community-based case management Experience with managing Home Care Packages and budgets is advantageous Current driver’s licence and a reliable vehicle with minimum Third-Party insurance Strong communication skills, both verbal and written Demonstrated empathy and compassion Excellent problem-solving abilities and critical thinking skills Good organisational and time management skills Cultural competence and the ability to work with diverse populations Advocacy skills to ensure clients' needs are met Collaboration and teamwork skills to work effectively with interdisciplinary teams Next steps: Please review the job ad carefully and, if you meet the criteria and are interested in applying, please seek endorsement from your current manager first before submitting your updated CV via the link (no email applications please).
