Company Description Job Description Located across the Eastern, Outer Eastern and Yarra Valley regions. Hybrid work model (home visits + office (Ferntree Gully) + WFH, where approved). Part-Time (0.8FTE) 12-month position – days negotiable. $44.78 - 48.99per hour + Not-for-Profit Salary Packaging to maximise your take home pay As a Care Partner, you’ll support older adults, their families and carers to access the right care and services, helping them live independently at home. You’ll build meaningful relationships, regularly connecting with clients over the phone and in their homes to understand their goals and ensure their support is tailored, responsive and aligned to what matters most to them. What you’ll be doing Conducting initial and ongoing assessments, developing personalised care plans Coordinating and managing services within individual budgets Supporting a range of needs including psychosocial, medical and physical Building strong relationships with clients, families and service providers Connecting with community networks to ensure the best outcomes for clients About you You’re an experienced Case Manager or Care Partner who enjoys building relationships and supporting older adults to access the care they need. You’ll bring: Min 2 years’ experience in Case management in home and community based settings A qualification in Aged Care, Health, Community Services or a related field (desirable) Confidence working with My Aged Care and external service providers Strong relationship-building skills with clients, families and stakeholders Knowledge of Aged Care Quality Standards and community care programs A current Australian Driver’s Licence Additional Information What's in it for you At Each you’ll be supported to be your best.
