* Own your portfolio, build real relationships — flexible, community-based case management * Hybrid and flexible work across home visits, WFH and office days * Access to a motor vehicle allowance to support client visits ($12,000 per annum) About the Role As a Care Partner at VMCH, you’ll own your own client portfolio, working closely with older Australians and their families to support independence, dignity, and wellbeing at home.  This is a flexible, community‑based case management role focused on building genuine relationships, with more time spent with clients and less time on admin and delivering high‑quality, person‑centred care coordination. Day to day responsibilities  * Deliver high‑quality, person‑centred care to clients in their homes and communities * Conduct comprehensive assessments and develop tailored care plans * Coordinate services and supports to meet individual client needs * Empower clients and carers to make informed decisions about their care * Ensure compliance with aged care quality standards and organisational policies * Maintain accurate documentation and uphold client confidentiality * Work collaboratively within a supportive team and escalate concerns when required About you Essential • Diploma qualification in a relevant field or higher (e.g. Advanced Diploma, Bachelor degree or above) • Experience as a Care Partner or Case Manager within Support at Home, Home Care Packages, or the Commonwealth Home Support Programme • Excellent communication and computer skills • Understanding of aged care reforms and quality standards • Ability to work autonomously and manage competing demands • Commitment to customer service, cultural diversity, and workplace safety • Willingness to travel as required and a current Victorian driver’s licence • Satisfactory National Police Check Desirable • Knowledge of Carer Recognition Acts and relevant service frameworks (e.g., HCP, CHSP) • Experience working with people from diverse backgrounds Why VMCH?
