Your Role as Care Partner As a Care Partner, you'll play a key role in helping older Australians remain independent, connected, and living well in their own homes. Working closely with customers, families, carers, and health professionals, you'll deliver person-centred care management that supports each customer's goals, preferences, and wellbeing. You'll deliver this role by: Developing, implementing, and reviewing personalised care plans that promote independence, wellbeing, and customer choice. Coordinating services and managing customer budgets within approved funding parameters. Building trusted relationships with customers, carers, families, and health professionals. Monitoring customer wellbeing, identifying changing needs, and responding proactively to potential risks. Applying wellness and reablement principles to empower customers to maintain their independence. Supporting customers to make informed decisions and navigate aged care services with confidence. Maintaining accurate documentation, care notes, and records in line with legislative and organisational requirements. Promoting safe, high-quality, culturally responsive care that respects each customer's individual needs, values, and preferences. What We’re Looking For We're seeking someone who is passionate about delivering high-quality, person-centred care and enjoys building meaningful relationships. You'll bring: Certificate III in Individual Support (Ageing), Certificate IV in Aged Care, or an equivalent qualification. Minimum 12 months experience in aged care, disability, community services, or a related sector. Excellent communication and relationship-building skills with customers, families, carers, and health professionals. Strong organisational and time management skills with the ability to manage competing priorities. Confidence using client management systems and other digital platforms. The ability to work independently while contributing positively to a collaborative team environment. A Valuable Addition, Yet Not a Must-Have! Experience in care management or case management. Knowledge of wellness and reablement approaches. Understanding of the Aged Care Act 2024 and Strengthened Aged Care Quality Standards. Experience supporting customers from diverse cultural backgrounds. Additional qualifications in aged care, community services, or a related discipline. Experience participating in quality improvement initiatives.
