We’d like you to join us as a Care Practitioner at our service Woodlands Neurological Rehabilitation Centre in York.
The purpose of the role is to ensure the safe running of the shift, working in partnership with the nurse in charge, engaging with care planning, managing the administration, storage and ordering of medications, supporting the MDT with the patients personalized timetabling, ensuring the environment is well maintained. To participate in auditing and implementation of change.

Responsibilities
  • To ensure quality care and Rehabilitation service which enables patients to be supported throughout their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety.
  • To work in support of qualified staff and allied health care professionals in the assessment, planning, implementing and evaluation of care without direct supervision of a registered practitioner.
  • To work as an Rehabilitation Team leader in ensuring that holistic care is delivered and standards of care are enhanced
  • To communicate effectively at all times within the team, other health care professionals and with the patient family, carers/ visitors
  • Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.
  • To regularly assess evaluate and report on actions and activities to registered practitioner and/or Multidisciplinary Care Team.
  • To supervise and delegate work to junior members of the team
  • Assess junior members of the teams skills and competencies and qualifications including NVQ’s.
  • Under go training and competencies that allow for the safe administration, storage and ordering of medication.
  • Under go training and competencies to ensure skills are developed and maintained to meet the needs of the service users.
  • To Regularly perform supervisions with junior staff to ensure staff competencies, expectation and standards are maintained.
  • Act as a preceptor for new staff members, offering excellent advice, support and guidance.
  • Support with the creation of Care plans, case review’s and discharge notes for services users specific needs.
