Legacy Oaks of Sacramento Assisted Living and Memory Care, is seeking a dedicated, compassionate, and detail-oriented Care Services Specialist to support, coordinate, and monitor exceptional care for the residents in our community. This role plays a key part in ensuring consistent, high-quality service delivery while supporting caregivers and maintaining regulatory compliance. Responsibilities of the Resident Care Coordinator Oversees caregiver and housekeeping scheduling, ensuring adequate staffing, real-time adjustments, coverage for call-outs, and minimal overtime. Provides daily supervision, coaching, and on-the-floor support to caregivers and housekeepers while reinforcing infection control, safety practices, and person-centered care. Conducts care rounds, environmental rounds, and quality audits to monitor resident wellbeing, cleanliness, safety, and compliance with Title 22 standards. Supports care coordination and service plan updates, collaborating with the RSS to prepare for care conferences, adjust ISPs, and communicate changes to staff. Manages documentation accuracy, auditing EHR entries (CARE/ALIS), housekeeping logs, competencies, and training records to support QAPI and survey readiness. Leads training and competency development, providing hands-on instruction, validating skills, and coordinating monthly in-services with the RSS. Assists with compliance, incident follow-up, and regulatory readiness, including investigations, corrective actions, and participation in licensing surveys. Fill in as needed to cover open shifts. Assist in daily requirements and tasks as requested. Participates in new employee orientation and monthly in-services. Ensure residents are provided privacy, respect, and dignity. Follows standard precautions and infection control procedures. Completes state required training upon hire and annually. Required Experience Possess, as a minimum, a high school diploma or equivalent. A minimum of two (2) years’ prior senior living experience preferred. A minimum of one (1) year prior Caregiver and/or Med Tech experience preferred. Be at least 18 years of age. Basic computer skills. Must be able to speak, read, and write in the English language.
