Care Coordinator
We are seeking an RPN to fill the role of Care Coordinator at our Memory Care Retirement Residence. This position reports to the General Manager and collaborates with the nursing staff to influence and advance the standard of clinical resident practice and care outcomes.
Responsibilities:
· Participates with the resident care team to improve clinical care practices
· Completes resident assessments as per schedule
· Fosters effective interpersonal relationships between residents, families, and other interdisciplinary health team members
· Works in collaboration with registered staff in the planning, developing and evaluation of nursing practice
· Accepts referrals from care team members and recommends referrals as necessary
· Ability to work on the floor and interact with residents, providing care and administering medications
· Acts as education resource to all members of the resident care team
Qualifications:
· Registered Practical Nurse with current certification of competence from the College of Nurses of Ontario
· Current membership with weRPN is desirable
· Has experience working in the retirement home sector
· Has experience working as a registered practical nurse in a managerial or supervisory capacity in a health care setting and
· Has demonstrated leadership and communication skills
