OVERVIEW

The Care Team Leader plays a vital role in providing high-quality care and support to individuals in various settings. This position requires a compassionate and dedicated individual who can lead a team of caregivers while ensuring the well-being and safety of service users. The Care Team Leader will be responsible for overseeing daily operations, implementing care plans, and fostering a supportive environment for both staff and Service users.

SKILLS

  • First aid certification and CPR training are essential.
  • Level 3 or working towards.
  • Strong patient observation skills to monitor changes in health status.
  • Knowledge of cleaning protocols to maintain hygiene standards.
  • Familiarity with home & community care practices is beneficial.
  • Experience assisting with ADLs and providing patient care is required.
  • Cooking skills for meal preparation tailored to dietary needs are advantageous.
  • Background in nursing home or long-term care experience is desirable.
  • Understanding of memory care principles for individuals with dementia or Alzheimer’s disease is a plus.
  • Knowledge of developing care plans that meet individual resident needs is important.

    Pay: £13.00-£18.60 per hour

Responsibilities
  • Supervise and support caregiving staff in delivering exceptional patient care.
  • Develop and implement individualized care plans tailored to each resident’s needs.
  • On Call Duties on alternate weekends
  • Must be flexible and reliable due to nature of the job
  • Monitor Service users’ health status, including vital signs and medication administration.
  • Assist Service users with Activities of Daily Living (ADLs), such as bathing, dressing, and meal preparation.
  • Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
  • Provide training and guidance to staff on best practices in caregiving, including first aid and CPR techniques.
  • Facilitate communication between Service users, families, and healthcare professionals regarding care needs.
  • Maintain a clean and safe environment for Service users doing spot checks and observations on care staff.
  • Participate in ongoing assessments of Service users’ conditions, including memory care for those with Alzheimer’s or dementia.
