Responsibilities:

SPONSORSHIP IS NOT AVAILABLE FOR THIS ROLE

Full time, 37.5 hours per week. Shifts to be worked over 3 days.
Days - 7.30am – 8pm / Nights – 7.30pm – 8am/ Including Weekends
Basic hourly rate - £13.57 per hour, up to £15.57 per hour with enhancement.
A night enhancement of £1.50 per hour will apply from 8pm to 7am Monday – Sunday (this does not apply to sleep-ins). A weekend day enhancement of £2.00 per hour will be paid from 7am to 8pm, Saturday and Sunday.
Oakwood is a rehabilitation unit for adults with Acquired Brain Injuries and associated neurological conditions based in Offerton, Stockport.

PURPOSE OF ROLE:

To work alongside therapists to support service users to develop and follow their individual rehabilitation programme. Maintain and update the Person-Centred Plans, promoting consistent service delivery in response to the changing needs of service users. Work under the direction of the Physiotherapist, Occupational Therapist, Speech and Language Therapist and Neuro Psychologist, working within the Service staff team.

KEY RESPONSIBILITIES:

  • Minute and report writing.
  • To organise and attend Multi-Disciplinary Team meetings and maintain accurate records, including minutes of meetings and agreed changes to Person Centred Plans/rehabilitation programmes.
  • Liaise with external health professionals and social workers, funders, Local Authority Staff etc.
  • Reviewing and updating Care Plans
  • To support the support workers to implement and support the therapy aspects of the service user’s rehabilitation programme, including challenging behaviour.
  • To make and record appropriate and necessary changes to the individual’s therapy programme within parameters, as agreed by our Physiotherapist/ Occupational Therapist/Speech and Language Therapist/Neuro Psychologist.
  • Chair Service User meetings.
  • To escort and assist service users in and around the home and to access community facilities as appropriate, including shopping, recreational, leisure and educational facilities as part of their therapy programme.
  • Supervising Support Workers
  • Involvement with new Admissions and discharge process, including set up of new care plans and discharge reports.
