Responsibilities

ROLE SUMMARY:

Care Workers provide high-quality, person-centred care and support to individuals in their own homes, promoting dignity, independence, and overall wellbeing. The role covers the provision of both Domiciliary Care and the Carers Break Support service, with the latter fully integrated into staff rotas and forming a standard part of the role. Care Workers deliver support as part of regular home care, as well as enabling unpaid carers to take planned breaks from their caring responsibilities. The role involves assisting with daily living tasks, personal care, and emotional support, while building trusting relationships with both those receiving care and their carers. Care Workers are expected to communicate effectively, respond to individual needs, and escalate any concerns to ensure safety and wellbeing.

MAIN DUTIES / RESPONSIBILITIES:

To visit and provide support to service users in their own homes, often visiting several service users during one shift.
To provide the support identified within their person-centred support plan.
To undertake domestic and cleaning duties in the home whilst supporting service users’ independence, as identified within their person-centred support plan.
To support service users with all activities of daily living and personal care including personal hygiene needs, nutrition, mobility and any other activity identified.
To administer or prompt prescribed medication as required, ensuring that Nurtrio’s medication procedures are accurately followed.
To undertake and support service users with shopping and cooking.
To be pro-active in supporting service users to engage with community activities, diversionary activities and therapeutic interventions to improve their health and well-being and reduce social isolation.
To be aware of responsibility to safeguard vulnerable people and promptly report concerns of abuse.
To use the electronic care planning system (Nourish) to record details of the support provided to the service user.
To both lone work and also work as part of a team as required.
The role requires flexible working, the hours will be generally within the hours of 7am – 11pm 7 days a week.
To undertake any other duties that are deemed necessary by the Team Coordinator/Senior Care Worker.
To have awareness and understanding relating to assistance technology and a willing to assist with its implementation.
To implement the principles of the Mental Capacity Act and support people to live a life of their choice.
Build and maintain positive relationships with carers and those receiving care, involving them in the care process, offering support when needed, and appropriately escalating any concerns to ensure their wellbeing and safety.
