Responsibilities: Provide personal care assistance including bathing, dressing, and grooming. Give medication reminders. Assist with daily living activities and mobility support. Engage clients in social and recreational activities to enhance well-being. Maintain a clean and safe environment for clients. Prepare meals and assist with feeding. Follow the Care Plan and document care activities reporting any changes in client condition. Maintain client safety and report changes in condition. Collaborate with HCA Manager to ensure continuity of care. Provide transportation for appointments and activities. Perform light housekeeping and laundry. Clock in and clock out. Build trusting relationships with clients and their families. Requirements: High school diploma or equivalent; State certification and license in good standing. Previous experience in caregiving or home health care is a plus. Familiar with Well Sky Personal Care app preferred. Strong communication skills and a compassionate demeanor. Ability to follow care plans and maintain client confidentiality. Current CPR and First Aid certification is a plus. Reliable transportation and a valid driver's license. Willingness to work flexible hours, including weekends and holidays. Must pass background checks and drug screening. Physical Requirements: Ability to lift, push or pull up to 25 pounds. Ability to assist with mobility and physical transfers. Ability to stand, bend and kneel for extended periods.
