Description:
Direct Work with Carers Work with individual teams to ensure carers are identified and offered appropriate support in accordance with the Care Act, 2014 requirements Connect carers to a range of relevant support, including use of community assets, voluntary or community services, and core commissioned services. Remaining in regular contact with Carers to gain formal and informal feedback on services. Ensuring Carers are aware of and invited to participate in engagement events and support groups. Promote carer self-management and independence by ensuring that carers receive the information and support they need to help build resilience, and enable them to manage their caring role.
Provide information on health promotion to carers, and ensure that any health and wellbeing needs are identified and appropriately address through sign posting to relevant services. To provide or sign post carers to information about mental illness, treatment and other relevant information and resources in the community. To liaise with other colleagues and organisations for and on behalf of carers and where relevant ensure carers are signposted to an advocacy service. To liaise regularly with Service Manager for Carers to ensure up to date information is being disseminated and seek clinical supervision with them to ensure professional development Ensure carers receive appropriate and timely information.To provide emotional and practical support to carers.
This will be achieved in various ways, but particularly by providing individual contact with carers and ensuring Carer Leads are doing the same. Promoting Carer Awareness Champion carer statutory rights and needs and ensure that any national and local information is cascaded to colleagues and local teams/service areas. Support initiatives aimed at promoting carer identification such as the Triangle of Care, Johns Campaign and the development of the Carers passport. Ensure the role of the Carer Champion is recognised and valued with mental health teams and services.
Ensure the role of Ward or Team Carer Leads are recognised and valued with mental health team and services. Ensuring Carer Leads are fully implemented within each team and ensuring Carer Leads are appropriately trained, informed and supported to fulfil their roles. Encouraging an environment and culture whereby all Services are aware of the significance and benefits of fully involving, including and supporting carers. Completing Carer Awareness training with staff members across all teams to enable staff to feel competent and confident to support carers
