General Description Under general supervision, provides non-medical, in-home support services to program participants and support to office staff and case managers. Maintains confidentiality and ethical standards as identified by NASW, HIPAA, or other relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma-informed care environment. Essential Duties and Responsibilities Looks at financial eligibility for the STEHP & ERAP program for all client referrals. Conducts intakes for emergency admissions Provides information and referral service for telephone inquiries. Maintains accurate activity records. Follows established procedures and policies for programs. Completes case notes electronically. Keeps accurate mileage and timesheet records. Utilizes web-based client database for client information and recording of units of service. Maintains up to date client service schedule on agency system (currently Outlook). Tracks any client payments received. Acts as liaison with other community service providers/referral sources and DSS after-hours workers. Supports department operations Back up for phones, report preparation, mail, and data maintenance. Administrative tasks such as copying, faxing, organizing, and chart maintenance. Assist with department meetings and events. Participates in relevant Agency meetings and/or trainings Actively pursues the development of professional competencies related to job role performance through reading, supervision, in-service training, and attendance at conferences and workshops. Awareness and active support of the Agency's Mission, Vision, Values, and Strategic Plan. Requirements Qualifications Education: Associate’s Degree in Human Services or related field preferred. Equivalent combination of education and experience will be considered Credentials: Valid and clean NYS Driver’s License. Experience: One year of experience in human services required. Experience in geriatrics preferred. Bilingual (Spanish, Burmese, Karen, Nepali, Somali or Arabic) desirable.
