Description
Description As a member of the Mariposa Heritage House team and a multidisciplinary service team, the Case Manager works with program participants to provide comprehensive case management services and coordinate care with community partners. The Case Manager supports individuals involved in recovery, behavioral health services, and community supervision by providing a single point of contact within the service system, helping coordinate care, remove barriers to services, and ensure continuity of support. This role connects participants with treatment providers, behavioral health services, housing resources, and other community support. The Case Manager provides advocacy and maintains open communication among program partners, including behavioral health providers, community organizations, probation, recovery programs, and other service providers. The Case Manager will take on responsibilities beyond the traditional “service broker” role by working collaboratively with partner agencies to support recovery, reduce recidivism, and improve participant stability. This position reports to the Program Coordinator. This position utilizes strengths-based, trauma-informed, and recovery-oriented case management practices consistent with SAMHSA guidelines. The Case Manager works to empower participants by helping them identify personal strengths, develop goals, and access resources that support long-term stability and recovery. The Case Manager will also support program staff with data collection, reporting, and evaluation activities to ensure compliance with grant and program requirements. Intake: · Conduct participant intake and orientation to services, including reviewing program expectations, identifying immediate needs, and providing crisis intervention when necessary. Assessment: Complete comprehensive needs assessments that may include: · Behavioral health needs · Substance use recovery needs · Housing and employment needs · Social support systems · Risk and protective factors · Assessments may include interviews, home visits, and coordination with community partners. Care Planning / Care Coordination: · Develop individualized case management and recovery plans that identify barriers and establish goals for participants. · Participate in multidisciplinary meetings and coordinate services with treatment providers, behavioral health partners, and community organizations. Referrals · Connect participants to appropriate services including: o behavioral health and substance use treatment o housing and homelessness services o employment or job training programs o mental health counseling o medical care and public assistance o recovery support services and life skills programs Intervention · Match available services and supports to participant needs and assist participants in engaging in services and making behavioral changes that support long-term stability. Monitoring · Maintain ongoing communication with program partners and monitor participant engagement and progress toward goals. · Monitoring may include reviewing service compliance, communication with partner agencies, and documenting progress. Evaluation · Evaluate participant progress toward service goals and determine whether services are effectively supporting improved outcomes. · Program evaluation may include: o service participation o program engagement o behavioral change indicators o recovery and stability outcomes o program compliance data Data Systems · Enter and maintain client data in program databases including: o Empower o Tyler Supervision o SPARS o GPRA reporting systems o Ensure documentation and reporting comply with SAMHSA grant reporting standards. Case Management Duties · Conduct intake and comprehensive needs assessments · Develop and maintain individualized service plans · Provide crisis intervention when needed · Coordinate services with partner agencies · Maintain timely documentation and service records · Prepare program reports as required · Attend multidisciplinary meetings and trainings · Participate in program evaluation activities · Maintain professional relationships with community partners Additional Responsibilities Administrative & General Duties Provide general support to shelter staff and program operations Answer multiple phone lines professionally Maintain and organize shelter forms, records, and documents Assist with data entry as needed Maintain and organize program supplies and communicate supply needs to supervisors Provide transportation as requested by supervision Perform other duties within job classification as assigned Requirements Education and Experience · Degree in Social Work, Human Services, Psychology, or related field preferred · Experience working with individuals impacted by substance use disorders, behavioral health challenges, housing instability, or justice involvement · Experience in case management, recovery services, or community-based programs preferred Additional Requirements Must successfully complete fingerprinting and background check upon hire (at Alliance expense) Must possess reliable transportation, a valid driver’s license, and DMV clearance Office equipment and general administrative functions Computer applications including Microsoft Office (Word, Excel, PowerPoint) and internet-based tools Social service organizations and service delivery systems Trauma-informed care principles Ability To: Work effectively in a fast-paced and sometimes stressful environment Communicate clearly and professionally, both verbally and in writing Exercise sound judgment and maintain confidentiality Work collaboratively and contribute to a team-based environment Build and maintain positive working relationships with clients, coworkers, and community partners Remain organized, reliable, punctual, and detail-oriented Physical Requirements Ability to see, read, and distinguish printed and handwritten documents Ability to hear and understand speech in person and by telephone Frequent use of hands and arms for computer and office equipment use Ability to lift and carry up to 20 pounds Ability to bend, stoop, walk, and reach overhead Ability to sit or stand for extended periods Ability to maintain concentration and mental acuity for sustained periods This list of essential functions and physical requirements is not exhaustive and may be modified as necessary in accordance with job requirements.
