Organization: SMCS-Valley Administration Position Overview: Coordinates and implements the transition of care (TOC) / Discharge (DC) plan for ambulatory patients. Prioritizes and coordinates the plan across the care of continuum through critical thinking, teamwork, and communication between care providers, patients, families and external vendors to ensure timely discharge. Job Description: EDUCATION: Equivalent experience will be accepted in lieu of the required degree or diploma. Associate's: AA in Business/Health related field TYPICAL EXPERIENCE: 1 year recent relevant experience. SKILLS AND KNOWLEDGE: Oral and written communication skills. Interpersonal and time management skills Ability to work effectively in a fast pace environment with rapidly shifting priorities and competing demands. Ability to work independently with a minimum of direction. Ability to exercise discretion and prioritize tasks, seeking input as indicated. Intermediate PC skills and word processing skills required. Pay Range is $28.80 to $37.43 / hour The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled.
