Day Shift - 7.5 Hours (United States of America) As a Case Management Assistant, you will facilitate the safe and timely transition from acute care to alternative levels of care such as skilled nursing facility, long-term acute care, inpatient rehabilitation, home infusion therapy, hospice and/or home care program. This position performs tasks identified in the inpatient discharge care plan for management of our patients across the health care continuum. Communicate with patients and all members of the health care team while coordinating services with post-acute providers and medical insurance payors.
