How you’ll make an impact in this role Care Orchestration: Design and manage comprehensive care plans across the continuum, proactively removing barriers to recovery and advocating for necessary resources. Clinical Oversight: Monitor medical necessity and appropriate levels of care through continuous assessment and medical record review. Strategic Collaboration: Maintain a constant feedback loop with supervisors and transition teams to refine care plans and ensure seamless handoffs. Resource Advocacy: Serve as a subject matter expert on documentation and post-discharge options, acting as a vital link between patients and post-acute service providers. Utilization Management: Partner with payers to optimize benefits and ensure the efficient, effective use of clinical resources. What minimum requirements you’ll need Licensure / Certification / Registration: Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date required. Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
