SKILLS & EXPERIENCE REQUIRED:

  • Extensive experience of working directly with mental health service-users and their carers
  • Experience of promoting independence through a strengths-based and empowering approach
  • Experience of support planning and achieving personalised outcomes
  • Strong understanding of how mental illness impacts individuals and families
  • Knowledge of key statutes underpinning adult social care, in particular the Care Act 2014
  • Observational skills to review and update assessed social care needs of vulnerable adults
Responsibilities

WHAT’S INVOLVED WITH THIS ROLE:

We are seeking to recruit a temporary Case Management Officer (CMO) to join our community mental health services. CMOs are based within multi-disciplinary teams and play a crucial role in maintaining flow through community mental health pathways.
CMOs carry a caseload of clinically stabilised mental health service-users, acting as the main point of contact for them and their unpaid carers. They are responsible for reviewing strengths-based care packages and supporting service-users through the transition from specialist mental health services back to the care of their GP. The role focuses on empowering service-users to optimise their independence and make best use of community resources.

KEY RESPONSIBILITIES:

  • Hold and manage a caseload of clinically stabilised service-users within the community mental health pathway.
  • Act as the main point of contact for service-users and their unpaid carers, providing advice, guidance, and support.
  • Review and update strengths-based packages of care to ensure they remain appropriate and effective.
  • Support service-users in their transition from specialist mental health services back to primary care under their GP.
  • Work within the framework of the Care Act 2014 and other relevant legislation, ensuring statutory requirements are met.
  • Undertake observations and assessments to identify changing social care needs and update support plans accordingly.
  • Maintain accurate case records, reports, and documentation in line with professional standards and policies.
