SUMMARY: St. Vincent de Paul Society of San Francisco Division Circle Navigation Center has been open for several years and will continue services for chronically homeless clients.  The Case Management Supervisor is responsible for ensuring that Case Management staff is working with clients to develop service plans that address their barriers to securing and maintaining housing, employment, and other vital necessities for self-sufficiency by adhering to the contract currently in place with the City and County of San Francisco Homelessness and Supportive Housing Department. This position is responsible for supervising staff and daily operations of the case management team at the Division Circle Navigation Center. This position may carry a small caseload, evaluate client needs, and develop a service plan for approximately 25 active clients. This is a full-time, exempt, non-union position working Monday - Friday, with occasional evenings and weekend commitments. SALARY AND BENEFIT OVERVIEW: The salary range for this position is $72,000 to $81,000 annually DOE. RESPONSIBILITIES: * Ensure the professional and sound operation of SF Adult Coordinated Entry Case Management Services at Division Circle Navigation Center, including regularly monitoring client activity related to achieving personal and programmatic goals by Case Managers, and providing corrective action revisions to case plans as needed.  * Coordinate staff and front desk scheduling and case management assignments; ensuring that the facility has consistent coverage of case management services 24 hours a day.  * Hire, supervise, and evaluate case management staff.  Supervise a case management staff with diverse areas of expertise including housing navigation services and placement, employment assistance, education, training, and other supportive services. Connect clients to Behavioral and Mental Health Services and other services.  * Hire, supervise, coach, and train case management trainees during a 3 month Case Management Trainee Program. During this program, trainee(s) will learn skills, resources, and techniques to work with clients while learning to maintain healthy boundaries with clients.     * Work collaboratively with Intake and Outreach staff, Director of Program Operations, Program Director, Site Manager, SFCE Case Manager City of San Francisco Department of Homelessness and Supportive Housing, and other agencies as needed or requested.  * Ensure program goals/targets and service deliverables are met on a monthly, quarterly, and yearly basis. Report progress every month. * Ensure client case management data and information is accurately entered into the ONE system, RTZ bed management, as well as to SVDP’s internal database system. Ensure information is accurate and readily available for review upon request.  * Analyze program data and plan for program improvements.  * Submit monthly reports with quantitative and qualitative information with support from the Site Manager, Program Director, and Director of Program Operations. * Review/Audit of client files (electronic) to ensure case management plans address identified needs, are up-to-date and that data quality is maintained.   * Attend program-related community, coalition, and committee meetings. Additionally, conduct or arrange for appropriate orientations, in-services, and trainings, for SF Adult Coordinated Entry Case Management staff.  * Maintain related administrative record-keeping and reporting systems as required by contracts. * Mediate client grievances and facilitate case management/client issues.  Ensure that Case Managers continually monitor and evaluate each client’s progression through their case management plan, developing corrective action revisions to the plan as needed.  * Maintain accurate and readily available documentation of service objectives and outcomes as well as other services while always being aware and in compliance with Federal, State, County, and SVDP guidelines.  * Assist clients in obtaining necessary resources to achieve case management goals. Actively participate in client and program-related meetings and trainings.  * Prepare case management-related reports including but not limited to: outcomes and successes.   * Adhere to strict boundaries and professional ethics in the care of others. Conduct crisis intervention as needed.  * Provide direct client services: individual counseling, case management, advocacy, and accompaniment as needed.  * Perform other appropriate duties as assigned. QUALIFICATIONS: * BA degree in social services, psychology, or another related field required. A master’s degree is highly preferred.  * Four years’ experience providing clinical case management services to chronically homeless preferred.  Relevant comparable experience will be considered. At least one year of supervisory experience is required. * Excellent proficiency in reading, writing, and speaking English. * Bilingual in Spanish preferred. * Ability to lead, motivate, and inspire staff. * Demonstrated skills in counseling, case management, and client advocacy. * Familiarity with the philosophy and approach of Harm Reduction, Motivational Interviewing, and OARS * Strong interpersonal, verbal, and written communication skills. * Understanding of homeless issues, especially in the context of race, class, gender, gender identity, and Sexual identity. * Understanding of the Housing First Approach to care for the homeless population.  Understanding of the Coordinated Entry system as it relates to Adult Coordinated Entry, Family Coordinated Entry, and Youth Coordinated Entry. * Knowledge, experience, and comfort working with a diverse community, including LGBTQQ, youth, immigrants, low-income communities, and communities of color. * Proficient in using MS Office including Microsoft Teams, Word, Outlook, and Excel. * Working knowledge of community resources.  Must be familiar with SF homeless shelter systems and services. * Demonstrated commitment to open, direct, and non-violent communication. * Ability to de-escalate crises and make decisions under pressure. * Excellent reporting skills including data entry, Excel, and Word processing. * Strong initiative and ability to work both independently and cooperatively with staff. * Ability to maintain professional boundaries with clients, volunteers, and staff. * Commitment to the mission of St. Vincent de Paul Society of San Francisco.   Covid vaccination and negative result on TB test required.    Background check completed upon acceptance of job offer.   St. Vincent de Paul Society of San Francisco celebrates diversity, equity, inclusion and belonging and is an Affirmative Action/Equal Opportunity Employer.   Pursuant to the San Francisco Fair Chance Ordinance we will consider for employment qualified applicants with arrest and/or conviction history.     Monday - Friday with occasional evening and weekend duties.
