About This Role Case Manager exists to build trusted relationships with members who need extra support navigating their healthcare and to ensure they never feel alone in the process. In this role, you will: Serve Redirect Health members within the 1% high-risk population, providing guidance, advocacy, and consistent clinical support Partner with members to create personalized care plans and proactively prevent avoidable hospitalizations and ER visits Directly impact lives by helping members access timely, coordinated, and compassionate healthcare This is a hands-on role for someone who: Enjoys connecting with people, solving complex care challenges, and guiding members through critical moments Takes ownership and follows through Wants their clinical experience to make a meaningful, human impact every day What You’ll Do In this role, you will: Own management of high-risk member populations, including monitoring, triage, and care plan execution Support members and families through education, outreach, and ongoing care coordination Collaborate with Medical Directors, Medical Assistants, providers, and internal care teams Improve Member outcomes by identifying early interventions and preventing unnecessary hospital utilization Advocate for members by navigating healthcare resources and ensuring continuity of care A strong performer in this role is known for: Strong clinical judgment and organization Communicating with empathy and confidence Consistently delivering timely, coordinated care that improves outcomes How Success Is Measured Success in this role is measured by: Reduction in avoidable hospitalizations and ER visits Timely development and execution of individualized care plans Positive member engagement and satisfaction Effective collaboration with clinical and operational teams What We’re Looking For We’re looking for someone who: Brings relevant clinical experience in case management, hospital settings, population health, or care coordination Has hands-on experience supporting patients with complex medical needs Communicates clearly and with empathy Takes ownership and follows through Enjoys simplifying complex medical situations Works well across teams and roles Cares deeply about helping people Background as an RN, LPN, paramedic, medical assistant, or other clinically trained healthcare professional ER, hospital, or high-acuity experience strongly preferred Case management experience preferred Why Join Redirect Health What “Free Healthcare” Actually Means When we say free, we mean $0 out of your paycheck and $0 when you need care: $0 monthly premiums $0 to add your spouse or children $0 deductibles (we reimburse them) $0 out-of-pocket maximums This benefit alone can save families tens of thousands of dollars.
