Salary $45,000

DESCRIPTION

Provide case management services to individuals and families residing in the Ozanam Family Shelter and Scattered Site PSH.

• Screening, assessment, service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers, life skills teaching, transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies.
• Utilization of HMIS case management software.
• Ability to work independently.
• On-call coverage and evening hours required.
• Bi-lingual encouraged to apply.

REQUIREMENTS

• Bachelor’s degree from an accredited college, preferably in Social Work.
• Experience with conflict resolution and mediation.
• 2-years relevant work experience and demonstrated ability to provide effective services to clients required.
• Needs to be culturally competent across all populations. Bilingual a plus.
• Driver’s license with acceptable driving history.
