Job DetailsJob Location: LaBella Campus - Middletown, CT 06457Position Type: Full TimeEducation Level: 2 Year DegreeSalary Range: $22.00 - $22.00 HourlyJob Shift: First ShiftPosition: Case Manager 1 Program: Middletown, Meriden, Wallingford Young Adult Crisis (MMWYAC) Location: Middletown Salary: $22.00/hour Schedule: Full time, 40 hours, 1st shift, Sunday-Thursday (8-4p & 9-5p) Program Summary: Providing crisis housing, case management, and other support to homeless young adults so they can maintain their safety while identifying long-term housing. Position Summary: This full-time position collaborates with the Program Director to ensure staff responds effectively to meet the needs of clients, maintains safe facility operations, and is responsive to community resources and funders. Provide daily supportive case management services to four (4) young adults between the ages of 18-24 and assistance to guests in a residential setting at the Eddy Shelter. Provide supportive case manager services and assistance to community-based clients and guests of the Eddy Shelter Rapid Rehousing Program. Provide assessment, comprehensive supportive services meeting basic needs of our shelter guests and community-based participants (for RRH only). Provide assistance through RRH for homeless individuals to ensure the safety, well-being, and progress towards goals that ultimately lead to ending their cycle of homelessness. Working within Adult Services, the Case Manager 1 works on the Behavioral Health Treatment care team to support individuals with severe and persistent mental illness residing in a residential setting. Requirements: Some Post-secondary education, Associate’s Degree preferred 2+ years related experience in the direct service and case management field Valid Connecticut Driver’s License Regular and predictable attendance is required Orientation: If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
