QUALIFICATIONS

  • Master of Social Work or equivalent in a mental health discipline and a minimum of three years’ related clinical experience or equivalent combination of education and experience
  • Membership in a professional College
  • Good verbal and written communication skills to provide therapeutic services to clients and families
  • Good problem-solving skills to analyze complex client issues and recommend solutions
  • Demonstrated skills in therapeutic engagement
  • First Aid/CPR is an asset
  • Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint)
  • Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work
  • Ability to work flexible hours, including evenings and weekends
Responsibilities
  • Provide developmentally informed transitional support for children, youth and families referred between SickKids and CCMH
  • Provide family education, counselling and support through brief interventions.
  • Facilitate multiple care aspects (e.g. case coordination, information sharing, implementation of practical clinical strategies, system navigation, caregiver support group).
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, patient and family centered and meaningful to the adolescent.
  • Develop effective working relationships with relevant hospital and community based mental health service system partners to collaborate/communicate throughout the entire case management process.
  • Practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice.
  • Prepares client documents and submits reporting information according to established procedures and deadlines.
  • Maintains up-to-date knowledge of therapeutic services and best practices within the industry.
