We are seeking a compassionate, organized, and client-focused Care Partner / Case Manager to join our dedicated home care team. This role is central to coordinating and delivering person-centred care to older adults and individuals living with chronic conditions or disabilities in the community. You will act as the primary liaison between clients, their families, healthcare providers, and support workers, ensuring all aspects of care are tailored, respectful, and empowering.

  • Certificate IV or Diploma in Community Services, Aged Care, Case Management, Social Work, Nursing, or related field.
  • Experience working in home care, aged care, community services, or case management (1–3 years preferred).

Experience working as NDIS Support Coordinator

  • Valid driver’s licence and access to a reliable vehicle.
  • Police check and Working with Vulnerable People clearance (or equivalent) required.
