Responsibilities

WHAT YOU’LL DO

We are seeking a passionate individual who will be working across two suicide aftercare services, the Suicide Prevention Service and the Way Back Support Service, the Case Managers/Support Coordinators are responsible for providing non-clinical care and practical psychosocial support to individuals in the first three months following a suicide attempt or suicidal crisis.
The service is based in Adelaide Hills around Mt Barker.

You’ll be trusted to make a valuable impact by:

  • Coordinating outreach/ home visits to provide practical and emotional support
  • Providing connection and referral to appropriate support services
  • Assisting clients and families in understanding suicidal behaviours
  • Developing clear goals with individuals and families and monitor achievements

To thrive in this role, you will have:

  • Tertiary qualifications in Social Work
  • Experience in the mental health field, or in case management
  • Effective communication skills and ability to engage with clients and families from a diverse range of background
