As a Case Manager – Out of Home Care (OOHC), you’ll play a critical role in supporting clients with complex needs through advocacy, referral pathways, and financial literacy education. Your opportunity You will provide case management services (across Tamworth and Armidale) that ensure client wellbeing and positive outcomes, while working collaboratively with carers and stakeholders. You’ll coordinate case plans, monitor progress, and maintain compliance with OOHC standards and legislation. This is a maximum-term part-time position working 6-hour shifts 3 days a week. The maximum term contract ends in June 2027. With integrity and resilience, you will: Deliver case management services including assessment, planning and intervention to support client safety and wellbeing Facilitate client visits, monitor quality interactions, and produce professional reports Recruit, assess, review and train carers to maintain required standards aligned with client case plans Maintain accurate and up-to-date client records in accordance with OOHC and agency requirements Engage stakeholders through positive relationships and clear communication of roles and responsibilities Monitor risks and progress towards agreed case plan goals with achievable outcomes Contribute to continuous improvement by assisting with policies, procedures and work practices Ensure compliance with funding body requirements, organisational policies, and evidence-based strategies Promote cultural awareness and respect, including consideration for Aboriginal and Torres Strait Islander peoples Participate in professional development, training, and safety initiatives What you’ll bring You are a compassionate professional who thrives on making a difference in people’s lives. You bring strong casework skills, excellent communication, and a collaborative approach. You will also need: Relevant tertiary qualification in Social Work, Psychology, or Welfare/Community Services Current NSW Driver’s Licence and WWCC Understanding of OOHC issues and relevant legislation Demonstrated skills in assessment, counselling, and casework Previous experience in a similar role Excellent written and verbal communication skills Strong analytical ability to apply legislation and policy to practice Intermediate computer skills, including MS Office Why Anglicare?
